Before you start
Have these details ready
- Primary contact and home address.
- Each child’s legal name, date of birth, and sex (up to four children).
- Insurance details or a clear front-card image—or choose self-pay or newborn coverage pending.
- A court order or adoption document if a listed contact is a legal guardian.
New patients must review and agree to the vaccination policy. The form does not ask for symptoms, diagnoses, medications, allergies, or medical history.

Your selected appointment details appear immediately on the final page. No confirmation email or confirmation number is issued. Print or save that page, and call the office to change or cancel an appointment.
